Specialist Person Centered Care

The Leonard Elms Nursing Home has been providing effective quality care to older people since 1993. Our services are responsive and are well led, offering specialist support and engagement to those with an enduring health condition.

The care team specialise in person centered dementia care, end stage dementia, complex nursing and evidence based clinical management of, disease, disorder or injury. Including end of life care.

Our home offers a safe, caring, homely and a secure environment, enabling residents to find their own way around with ease, comfort and independence.

We are proud of our positive culture and values and support anyone whatever their background or nature of their condition.

Recreation and Activities

A range of activities are on offer to suit each individual residents needs, we help you follow your hobbies and interests and we facilitate social engagement and help organise recreational events. Residents can have access to our wonderful mature gardens and courtyard that offer attractive features and wondering space. So come and enjoy the wild life found in our beautiful Countryside setting.


A modern purpose built home that provides a choice of ensuited bedrooms offering light and airy accommodation, fully furnished with multi positioning beds, nurse call points, media and WiFi access, assisted bathing facilities and a choice of stylish air conditioned lounges for formal and informal dinning and relaxation. The environment is Dementia appropriate and offers security and peace of mind.

Good food - an important part of our lives

For many of us, the enjoyment of good food is an important part of our lives. Our team of chefs do their upmost to accommodate our residents wishes through the preparation of attractive meals using the freshest of local ingredients. The menu choices change each season and include many traditional dishes. We cater for those with special dietary needs and specific clinical nutritional requirements.

Specialist Dementia Centre

The home provides specialist dementia services with staff training derived from the highly regarded dementia service development centre at Sterling University. Some care staff have achieved "Best Practice Awards" from the University. The Home has a Dementia Lead Nurse who focuses on best practice in this area and can assist social care professionals to place and formulate complex care plans.

Palliative Care Services

Palliative care is an approach that improves the quality of life of our residents and their families facing the problems associated with life -threatening conditions. Our aim is to improve the quality of life of both resident and family helping the individual to live as well as possible until they die, and die with dignity, providing relief form symptoms, pain and stress of a serious illness.

General Nursing Care

Treatment of disease, disorder or injury accredited nursing care services for those with long term conditions; higher clinical care and personal support needs.

Specialist Dementia Care

Optima Care have specialist nursing services with specially trained staff that understand the challenges associated with complex dementia conditions; helping those who are distressed or at risk of harming themselves.

End of Life Care

Our care homes offer palliative care, where a dedicated team of nurses provide around-the-clock care in a calm and comfortable environment for every resident. We understand that end stage illnesses affect everyone differently, so we work with you to tailor your loved one’s individual care needs.

Katie Maidment

Registered Care Home Manager

Katie first worked at The Leonard Elms in 1995 as a care assistant. Leaving to undertake her diploma in adult nursing, Katie soon established herself as an accomplished NHS community nurse in Worle.

Ed Bliss

Dementia Lead Nurse

Ed joined Optima Care as a Care Assistant in 2010 where he found his interest in dementia care. Qualifying in 2017 with a BSc Hons Degree in Mental Health Nursing, Ed now specialises in mental health care.
